This template is used to define, approve, test, and evidence monitoring, logging, and evidence requirements for an AI use case.
AI systems must be observable and reconstructable.
The purpose of this template is to ensure the enterprise can understand what AI saw, what it produced, what data it accessed, what tools it called, what actions it triggered, who reviewed or approved it, and how evidence is retained for assurance, audit, investigation, and incident response.
